- Please do not attend if you:

Please contact the surgery prior to your appointment to discuss these situations. - All entering the surgery need to check-in upon arrival:
– scanning the provided QR code,
– presenting their personal COVID-19 check-in card (which we can scan), or
– we can manually enter your details into the NSW Health website Concierge Form. - Please use the alcohol-based hand sanitiser provided as you enter reception and/or wash hands with soap.
- Observe social distancing measures in the practice premises.
- Masks should continue to be worn into the surgery – while in the reception area, and until seated in the surgery.
- We ask that the number of persons attending be kept to a minimum – preferably only the patient to be seen, but accompanying carer or parent/guardian may also attend if healthy. Accompanying persons must continue to wear their mask while in the practice premises whether in reception or if accompanying patient into the surgery.
